Using diagrams to explore opportunities in sustainability and scalability for an aging in place national nonprofit

CAPABLE is an evidence-based client-centered home-based intervention for aging, developed at John Hopkins School of Nursing. CAPABLE programs are implemented all across the U.S. and Canada.

In my Communication Design Workshop at the Institute of Design, we are working with CAPABLE to see how visualizing complex can lead to insights for scalability. How can an emerging, evidence-based program that is showing success—like CAPABLE—grow?

This ongoing initiative, set to conclude in May 2025, will transform our insights, findings, and explorations into a series of shareable visual diagrams and design artifacts for the CAPABLE National Center.

So far, we’ve been conducting in-depth interviews with stakeholders at all levels—from frontline nurses to national leadership—to systematically map and visualize the CAPABLE ecosystem, which varies across different contexts.

The diagrams below, hand-sketched by me, are derived from both my own interviews and those conducted by the class. They serve as a tool for synthesizing complex information and uncovering patterns within our collective data. Stay tuned for the final outcome in May!
